The Leisure Needs Survey asks about facility usage and satisfaction, frequency of participation in leisure activities, and the importance of MWR programs. Your feedback will help us gain insight into our quality of life programs and determine where we can make improvements to serve the military community better. 

How to access the survey 

The Fort Greely Family and MWR Leisure Needs Survey closes Monday, November 22, at 11:59 p.m. AKDT. 

1. Paper Format: Available at all Family and MWR facilities.
2. Fillable PDF Format:
